To ensure that the surf session on the Citywave will be an unforgettable experience, we kindly ask you to read the terms and conditions carefully and sign below.

I am aware that surfing on the Citywave is a physical activity and that participation involves a risk of injury. My child takes part at their own risk.

Non-swimmers are not permitted to use the facility!

For participants under the age of 18, a parent or legal guardian must sign this form.

YES, MY CHILD MEETS THE FOLLOWING participation requirements of the Citywave:

My child's weight does NOT exceed 120 kilograms.
My child has NO shoulder or joint problems.
My child has NO back, disc, or spinal problems.
My child has NO cardiovascular disease.
My child does NOT have a pacemaker.
My child has NOT suffered a stroke.
My child is NOT under the influence of alcohol or drugs.
My child currently has NO acute injury.
My child is NOT suffering from any acute illness.
My child has NO disease of the brain, balance organ, or nervous system.
There are NO other physical conditions that Citywave (AWSM GmbH) should be informed of prior to the surf session.
My child is OLDER than eight years.

I am aware that by using the facility, my child automatically gives consent to the free recording of photos, live images, and videos.

I am familiar with the full terms and conditions and confirm the above statements for my child.
